Duck River Dental Outreach provides comprehensive dental services for low-income, uninsured individuals suffering from devastating dental disease on a sliding fee scale based on income and family size.

When you call to inquire about becoming a new patient, our exceptional team of Patient Care Coordinators
will ask you for some basic information to help route you through our program to address your dental needs.
Applying for the Duck River Dental Outreach Comprehensive Care Program
Applications for this program are available on a first come first-served basis. Both the Family Member Information and Clinic Guidelines forms must be completed.

Patient Care Coordinators at (931) 388-6410 to find other ways to obtain an application.
