Contributor Level $25+ - can provide one comprehensive evaluation, including X-Rays, to enroll a patient into our program

Associate Level $100+ - can provide 4 evaluations or extractions of severely decayed, infected, and painful teeth

Advocate Level $500+ - can provide any combination of the above plus provide needed medical equipment, supplies, or working capital

Patron Level $1000+ - can provide one crown or one partial denture

Champion Level $2000+ - can provide one complete upper or lower denture

Dr. Tom Moore Benefactor Circle $5000+ - can provide any combination of the above plus funding of needed medical equipment and/or a donation to our building fund
